How do I change my shower head with round bulb underneath?

I took off the current shower head and underneath it is a round bulb. None of the how-to videos that I have watched show a round bulb underneath the shower head so I do not know what to do. I tried to unscrew the round bulb, but it is on very tight, and I do not know if it supposed to come off.

The hole in the bulb is to reduce the size of the pipe to add water pressure to the head. Just install another head.
